Botox is one of the most well-known and widely used injectables in cosmetics. If you’ve looked in the mirror recently and noticed lines starting to develop in your forehead or between your eyebrows, you might have felt like they’ve caused an aged or tired look in your face. Although these symptoms of aging are completely normal, they can impact our confidence and make us look older.

The good news is, you are not alone! Thousands of patients experience these issues and can effectively address them with Botox injections.

At Associates in Plastic Surgery, I tell all of my patients that Botox is one of the best and simplest things they can do for themselves cosmetically. It’s quick, it’s effective, and it is exceptionally discreet.

I’m Julie Miller, M.D., Ph.D., a board-certified ophthalmologist and facial aesthetic specialist here at Associates in Plastic Surgery. My extensive background and advanced training have given me a great understanding of facial anatomy, allowing me to give my patients the best results with Botox.

Why I Relate to My Patients

I love Botox for its ability to deliver subtle and impactful results—I even use the injectable myself and get regular treatments.

As a 55-year-old woman, I understand firsthand what it feels like to look in the mirror and notice changes in your face. It’s only natural that we focus on lines that weren’t there before or other changes that make us look more tired than we feel.

When it comes to Botox, I can speak to my patients not only as a physician, but as someone who gets the injections personally. I know how they feel, what the process is like, and what great results are.

So when I design a treatment plan, I’m combining medical expertise with personal experience. That perspective allows me to approach every consultation with a unique understanding that not all injectors carry.

Before and After Photos

What Is Botox?

Botox is a neurotoxin injectable, meaning it works by relaxing the facial muscles it is injected into. By relaxing these facial muscles, we can smooth out the skin, preventing deep wrinkles from appearing or worsening. Botox is often confused with fillers, which add volume; Botox does not add any volume to the skin; it only relaxes the muscles.

Botox has several other uses, which we will discuss later on this page.

Common treatment areas include:

  • Forehead lines
  • Frown lines (the “11s” between the eyebrows)
  • Crow’s feet
  • Brow lift
  • Under-eye softening
  • Lip flip
  • Chin dimpling
  • Jaw tension

Why Botox Is So Effective

Botox’s power is in its subtlety. Rather than create intense changes, Botox works beneath the surface to provide its results. It starts working to relax the muscles immediately, with results appearing gradually after just a few days.

The appointments are quick and only take about 10 to 15 minutes. After injection, patients can go about their day as normal. It is a great alternative to surgery and can even act as a form of prejuvenation, which is when younger patients receive the injectable to prevent wrinkles from forming in the future. Results with Botox are consistent and always very impactful.

Natural Botox Results: Don’t Want a “Frozen” Look?

Some patients want to achieve more obvious results with Botox, which is totally fine. However, some patients want results that are very subtle; they want to be able to move their faces after their injections. This can very easily be achieved at my office with careful Botox injection. By carefully injecting the Botox into targeted regions, we can allow for natural eyebrow movement, forehead motion, and soft results that look completely natural.

When Will You See Results?

An important thing to know about Botox is that results are not instant. The formula indeed starts working immediately to relax the muscles, but you won’t see results until a few days later. Some can see changes in as little as 3 days, but most patients notice more significant results after about 5 days. This is still very quick, especially compared to the downtime patients have to go through with surgical procedures before seeing their final results.

Full results from Botox typically develop after 10 to 14 days. My patients are regularly very impressed with their results, especially given how subtle yet impactful they are.

A lot of my patients say that people can’t even tell they had Botox, they just think they look great!

Botox vs. Other Neurotoxins

Today, there are a lot of neurotoxin injectables on the market. Botox remains one of the most popular and well-known brands, but the truth is that they all work very similarly, with only minor differences. Instead of focusing on the brand name, I tell my patients to focus on an injector’s:

  • Experience
  • Anatomical knowledge
  • Precision
  • Conservative dosing

After all, technique is everything!

My Unique Background: From Medical Botox to Cosmetic Excellence

Many people might not know that Botox has uses other than cosmetics. It was actually used for mostly medical applications before it became more widely known for cosmetics. We ophthalmologists were among the first specialists trained to use Botox for therapeutic indications.

Botox was originally used to treat:

  • Blepharospasm (involuntary eye muscle spasms)
  • Strabismus (misaligned eyes)
  • Facial tics involving the areas around the eyes

Because these treatments dealt with functional aspects, they required a lot of precision, considering the delicate and complicated nature of the eye muscles.

My early training for these indications really impacted how I view Botox injections and how I carry out injections today.

When Botox transitioned into aesthetic medicine, I already understood:

  • Facial muscle dynamics
  • Subtle muscle balance
  • The importance of symmetry
  • The consequences of over-treatment

I later pursued fellowship training in cosmetic injectables to refine my techniques.

Botox: Not Just Superficial

As previously mentioned, cosmetic injections, especially Botox, are not just superficial.

Botox continues to have many medical uses, many of which I have a unique background in, including:

TMJ and Jaw Tension

By relaxing overactive jaw muscles, Botox can reduce clenching, grinding, and the discomfort associated with TMJ. As a result, patients are able to benefit from reduced jaw pain, fewer tension headaches caused by jaw clenching, and a reduction in nighttime grinding.

Migraines

Not only can Botox relax the muscles that cause wrinkles, but it can also prevent pain-causing neurotransmitters from being released—specifically, the ones that cause migraines.

Regular Botox injections are approved for chronic migraines by the FDA. This procedure can greatly reduce the occurrence and severity of migraines. In this way, Botox has improved the quality of life for many patients and is a reliable way to find relief from chronic migraines.

Hyperhidrosis (Excess Sweating)

Hyperhidrosis is characterized by overactive sweat glands, causing patients to sweat even when they are not exercising or warm. This can cause discomfort, excessive odors, and general self-consciousness.

Injecting Botox into the armpits can reduce sweating greatly, allowing patients to find relief from this condition and have dry underarms. I remember one patient I had, a young woman who was deeply embarrassed by her underarm sweating. After treatment, she was able to feel more confident in her daily life, and now she only needs injections about twice a year to maintain her results.

What to Expect at Your Appointment

Corresponding Treatments injectable facial fillers laser treatments

When you come in for your consultation to discuss goals and expectations, we may be able to schedule your Botox appointment that same day. After a facial muscle assessment, I will design a customized injection plan.

Treatment will be only 10-15 minutes, allowing you to return to your normal routine right away.

There is no required downtime, although you might feel some slight tenderness in the treated areas. Many of my patients schedule their Botox injections on a Friday and return to work on Monday looking refreshed!

How Long Does Botox Last?

On average, Botox lasts 3 to 4 months for most patients or sometimes slightly longer with consistent treatment; over time, some patients notice they need fewer units because the treated muscles become trained to relax.

Many of my patients schedule maintenance treatments about 3 times per year to maintain consistent results. Botox can have a much longer-lasting impact if patients get regular treatments, as patients who use the injectable as a form of prejuvenation can prevent certain wrinkles from forming or worsening.
